Immunization Requirements

California law requires specific health screening for all children entering school for the first time.

The School Entry Health Exam Requirement Form must be completed by a pediatrician within 18 months before first grade or up to 90 days after starting first grade. This can be done at the time of a scheduled physical exam. The completed for must be returned to the school office.

The Oral Health Assessment Form states that children must have a dental check-up by May 31 of the first year in school.

California law requires that students entering 7th grade have received the Tdap booster shot.
